If you're Filipino and employed, chances are your PhilHealth was set up for you by HR and you've never had to think about it. But if you're a freelancer, newly self-employed, a stay-at-home parent going back to work, or just someone who's never had a PhilHealth ID at all — this one's for you.

I broke the whole process down into plain, simple steps: no government jargon, no confusing forms, just what to actually click and bring.



Step 1: Figure Out Your Membership Type

Before anything else, PhilHealth needs to know which category you fall under, since this decides what documents you'll need:

  • Employed — your employer typically handles registration for you
  • Self-employed / Freelancer — you register and pay on your own
  • Voluntary member — for those not currently working but want coverage
  • OFW — a separate registration track with different documents (passport, work contract)

Most first-time DIY registrants fall under self-employed or voluntary. The rest of this guide follows that path.


Step 2: Prepare Your Documents

Before you open your browser, get these ready so you're not scrambling mid-registration:

  • A valid government-issued ID (National ID, passport, driver's license, UMID, etc.)
  • Your PSA birth certificate (for brand-new registrants)
  • Proof of income, if you're self-employed (this can be a simple declaration for voluntary members)

Tip: Scan or photograph documents clearly. Blurry uploads are one of the most common reasons applications get delayed or rejected.


Step 3: Download and Fill Out the PMRF

The PhilHealth Member Registration Form (PMRF) is the foundation of your application.

  1. Download the latest PMRF directly from the official PhilHealth website
  2. Fill it out in capital letters
  3. Leave the 12-digit PIN field blank — this is only for existing members updating their record; PhilHealth assigns your PIN after processing
  4. Double-check your name, birth date, and address exactly as they appear on your PSA birth certificate — even a missing middle initial can cause a mismatch error later

Step 4: Register Online

  1. Go to the official PhilHealth website and verify the URL starts with https:// with a lock icon — never use third-party sites claiming to register you
  2. Look for "Online Services"  and select "Register/LogIn" - it will take you to another page, find and click "Philhealth Member Registration"
  3. Create your account with a valid, active email address and enter the CAPTCHA. Verify your email by checking your email account. 
  4. Open the PIN Application form

Click the Register/LogIn link.


Choose Philhealth Member Registration


  1. Fill in your personal details — full name, birth date, sex, civil status, and address — exactly as they appear on your PSA birth certificate
  1. Select your membership type (employed, self-employed, voluntary, or OFW) — this determines which additional fields and documents you'll be asked for next
  1. Upload your scanned PMRF, valid ID, PSA birth certificate, and proof of income
  2. Tick the data privacy agreement box
  3. Review everything carefully, then hit Submit



Step 5: Wait for Your PIN

After submitting:

  • Check your email — an acknowledgment usually arrives within minutes (check spam if it doesn't show up)
  • Processing typically takes 3 to 5 working days
  • Once approved, PhilHealth sends your permanent 12-digit PhilHealth Identification Number (PIN) in a separate email

This PIN is yours for life — it's the same number used across every PhilHealth transaction, no matter how many times you change jobs or membership type.

Step 6: Know What You'll Be Paying

For 2026, the PhilHealth premium rate is a flat 5% of your monthly basic salary or declared income, with:

  • Income floor: ₱10,000 (minimum monthly premium: ₱500)
  • Income ceiling: ₱100,000 (maximum monthly premium: ₱5,000)

If you're employed: this is split 50/50 with your employer — you each pay 2.5%.

If you're self-employed, voluntary, or an OFW: you shoulder the full 5% yourself.

Example: If you declare ₱20,000/month, your total premium is ₱1,000. As an employee, you'd pay ₱500; as a self-employed or voluntary member, you'd pay the full ₱1,000.

Before paying, note that PhilHealth now requires you to generate a Statement of Premium Account (SPA) first — payments made without one may be rejected by GCash, Maya, or partner banks.


Step 7: Get Your Physical ID or MDR

Once your PIN is active, you can:

  • Log in to the Member Portal anytime using your PIN to view your Member Data Record (MDR) — this shows your PIN, contribution history, and listed dependents
  • Download and print your MDR as a PDF to keep on hand for hospital visits
  • Some branches also issue a physical PVC PhilHealth ID card, which doubles as a valid ID for certain transactions like GCash or Maya verification



A Few Things I Wish I Knew Earlier

  • Don't start a duplicate registration if you've simply lost your PIN. It's permanent — you just need to recover it, not reapply.
  • Add your dependents right away. If they're not listed on your MDR, they won't be covered when you need to use your benefits.
  • Keep a printed MDR copy at home. Hospital systems go offline, and you don't want to be hunting for Wi-Fi during an emergency.
  • Double-check everything before submitting. The portal's error messages aren't very helpful, and a single typo can delay your PIN by days.
