Raise your hand if this has happened to you: you're at the hospital, or filling out a form, or your HR is asking for it — and suddenly you cannot remember your PhilHealth number to save your life. 🙋‍♀️

The good news? Your PhilHealth Identification Number (PIN) is permanent — you don't lose it, you just misplace it. Here's every legit way to track it down again, from fastest to most old-school.

Here's the catch a lot of people run into: PhilHealth's Member Portal actually uses your 12-digit PIN itself as your login username. So if you've genuinely forgotten your number, logging into the portal isn't an option — you'll need to recover it through one of the methods below first. Once you have it, you can log into the portal anytime to view your MDR, check contributions, and manage your account.

Method 1: The PhilHealth Check Utility (New — facial verification)

This is PhilHealth's newer online tool for members who don't have Member Portal access yet. Instead of a password, it verifies you through your personal details plus a quick facial "liveness" check.

  1. Go to the official PhilHealth Check Utility on philhealth.gov.ph
  2. Enter your personal information exactly as it appears on your PhilHealth record (not necessarily your current legal name if it's changed — use what's on file)
  3. Allow camera access when prompted
  4. Follow the on-screen movements for the liveness check (this confirms you're a real person, not a photo)
  5. Once matched, your membership details and PIN will display

Tip: Do this somewhere well-lit and remove face masks, sunglasses, or anything covering your face for the scan to work smoothly.


Method 2: The eGovPH App

If you've already linked PhilHealth to the eGovPH app, your PIN and MDR should be accessible there too — handy since it's the same app used for other government IDs.

  1. Open the eGovPH app
  2. Go to the PhilHealth section
  3. Your linked membership details, including PIN, will show if previously connected

Method 3: Email Inquiry (No online account needed)

Don't have online access at all? You can request your PIN directly:

  1. Email the official PhilHealth inquiry address (check philhealth.gov.ph for the current one, as this can change by region - actioncenter@philhealth.gov.ph as of today, 6 August 2026.
  2. Include your full name, birthdate, and a clear photo of a valid ID
  3. Use the subject line: "PhilHealth Number Verification"
  4. Wait for their reply — response times vary, so this isn't the fastest option, but it works

Method 4: Call the Hotline

For a real-time option without needing to go anywhere:

  • Contact the official PhilHealth hotline (listed on philhealth.gov.ph)
  • Be ready to verify your identity with basic personal details
  • Fair warning: hold times can be long, so this works best if you're not in a rush

Method 5: Visit a Branch (The surest, slowest option)

If all else fails — or you just prefer doing things in person — bring a valid government ID to any PhilHealth branch and request verification over the counter.

A Few Things to Keep in Mind

  • Data privacy matters here. You generally can't just look someone up by typing a name — PhilHealth requires identity verification before releasing any PIN. This protects your records, so don't be surprised by the extra verification steps.
  • Use the name on your PhilHealth record, even if it's since changed (maiden name, for example) — mismatches are the most common reason verification fails.
  • Save your MDR once you find it. Screenshot it, save the PDF, and keep a printed copy somewhere accessible — you really don't want to be searching for Wi-Fi during a 2 AM emergency room visit.
